Application Check List :

1. Please save the below preference Language application form :

a. Click here to download Malay & English version.

b. Click here to download Chinese version. ( To  open the chinese version applicaiton form, your PC must have chinese font.)

2. Click Here to download  and read FLP's ' Company Policies ', which are published and available at www.forever.net.my.

3. Please fill in the information requested in each of the sections.

4. Save the file using your full name as the filename and email it to join@forever.net.my together with the clear copy/image of :

a. Your NRIC/Passport.

b. The information page of your bank passbook/bank statement.

c. The bank-in slip/transfer transaction receipt for RM95 for the purchase of the Starter Kit.

Note : If you do not attach the bank-in slip for the payment of RM95 with your application, the application will not be processed.
